Forms

Volunteer National Police Certificate (VNPC) Application Form

Please complete the application form and submit to the parish for Section E to be completed by the authorised parish representative (parish priest, locum, warden or parish administrator), together with 100 points of ID.

  • The parish priest or authorised parish representative will forward the completed form to Safe Ministry Services for processing.

Please note: All documentation must be kept for two years and is stored securely.

    Adverse Finding

The parish priest will only be notified of the outcome of a VNPC application if an adverse finding is returned.

If the finding is considered to be serious and places people at risk, the Safe Ministry Services Manager (SMSM) will contact the parish priest and ask for a full disclosure to be made.

If the information in the disclosure identifies that the person is not safe or is a known sex offender, the SMSM will assess the risk and manage appropriately.
